The BFAWU National Health and Safety Committee has decided that there is a need for a consistent approach to the recording of both hazard reporting and near misses. Click here to download the Hazard/Near Miss Reporting Form that should be completed whenever a hazard or near miss are reported. These should then be retained in a secure place as they may be used for future reference in the event of an accident, or in health and safety surveys.

By using the template, we ensure that we get information recorded in a consitent way irrespective of the Branch or its location. These forms should be kept in a secure location to assist with future reference to improvements to be carried out, or should recommendations be ignored.You should print copies of this ofrm every time you or a fellow Representaive are arrying out an inspection, and should make them available for Safety Committees.
